Caring, compassionate, and competent describes our experience at The Park Ave Extended Care Facility. After suffering a stroke at the age of 86, my mom lost her ability to walk. From the moment we arrived at the facility, we where greeted be a kind and courteous staff, sparkling clean facility, and top notch medical assessment and treatment teams.The talented and skilled men and women , assessed her condition and proceeded to address every one of my mothers medical conditions, identify and treat all of them simultaneously and it worked! She recovered her ability to walk! Every aspect of her stay there was carefully planned and customized to my mom individual needs. The entire staff worked as a team to provide the best care possible. From admissions to administration, the treatment team, housekeeping, all of these professionals are the best in their field. I truly thank all of them for giving my mother her life back."

In 2004 after having surgery on both legs to remove blood clots I came to Park Avenue Extended Care for rehab. Their rehab department had me on my feet and walking in a short time. I also have a heart condition that prohibits me from living alone. Park Avenue's Doctors and nursing staff have kept me in reasonably good health for the past ten years.

In the time I've been here I have come to realize that I am in a first class nursing facility. In addition to nursing and rehab they keep us comfortable and entertained. Each room is beautifully decorated and has a flat screen television with more than four channels. The Recreation department provides daily activities and weekly parties with live entertainment and a weekly summer Bar-BQ's on our large beautiful patio.

I am proud to call Park Avenue Extended Care my home. When the time comes I hope my obituary will say that I died peacefully at my "HOME" at Park Avenue Extended Care."

As a resident here for these last five years, I would like to share my thoughts with you. Park Avenue Extended Care is my home in so many ways; my bed is very comfortable, my meals are tasty and plentiful, my room is kept so clean and most of all, the staff of doctors, nurses and aides are very caring and qualified. By the way, the d├ęcor, on all eight floors as well as the lobby is charring. If I wish, I can keep busy by playing bingo, poker, arts and crafts, trivia pursuit or just being entertained very often by musical programs that are delightful. Otherwise, when I'm just lazy, I can go to my room, relax, and watch my favorite television show, I have made many friends here and I consider myself lucky to have found contentment where my family is happy for me too."

When you enter the sliding doors of Park Avenue Extended Care you'll see a clean spacious lobby with couches and a television so that residents, family and friends can visit or enjoying the television. On Saturday and Sunday you can enjoy free coffee and cookies. Every month we have get together on the eighty floor for Birthday parties with musicians who make us feel young again. We have weekly summer Bar-BQ's every Wednesday out on the patio. So many of our residents attend and enjoy the food and fun. There are plenty of activities to keep us busy and all of us are being taken care of in a clean, sunny and caring home away from Home."
