Park Avenue Extended Care is dedicated to providing outstanding rehabilitation services in an environment that's comfortable, immaculate, and compassionate. Our newly renovated rehabilitation department offers an activities and daily living suite as well as the latest in all state of the art equipment. Our unit is bright, crisp, clean, and welcoming, the perfect environment for recovery. Our skilled team of nurses, therapists, physicians, and staff will make you feel "right at home" while you are recovering. At Park Avenue Extended Care our goal is to assist our residents in regaining their optimum physical health through an interdisciplinary team approach.
Short term residents recovering from an accident, injury, surgery or illness learn to regain skills in bathing, cooking, stair climbing, transferring in and out of bed, laundry and other activities of daily living. Park Avenue Extended Care offers a complete tailor made rehabilitation program for each resident designed by our staff of nurses, therapists, and physicians.
Our short term residents reside on the 8th floor with views of the beautiful ocean and bay. All residents are invited to socialize in their community room, (centrally located on every floor), or the sunlit lobby with flat screen television and baby grand piano.

Accessible from a wheelchair or transfer from a bed. This equipment can be used to improve muscle strength of the lower extremities. The Moveo combines the benefits of traditional tilt table standing with active exercise. This combination allows patients who are incapable of supporting full body weight to participate in graded weight bearing activity at an earlier stage in the rehabilitation process.
Physical therapy is a treatment method for improving limited body movement and functionality that are a result of disease, injury or aging. Treatment involves restorative exercises that focus on developing muscle strength, flexibility, balance, posture and coordination, and that provide overall pain relief. Physical therapy is designed to promote a patient's overall health and fitness, prevent re-injury, and maximize quality of life. P.T. may be prescribed as an initial form of treatment for certain conditions or injuries, or to restore strength and function after surgery.
Occupational therapists assist individuals with mental, emotional, physical or developmental disabilities in carrying out day-to-day tasks. They provide disabled individuals with customized techniques for eating, cooking, dressing, using the computer, and other activities. Where necessary, occupational therapists teach individuals how to use wheelchairs and other adaptive equipment to help them overcome their disabilities.
Speech disorders may develop later in life as a result of trauma or injury. Speech therapists are specially trained in helping individuals correct speech disorders and speak clearly. Regular sessions with a qualified speech therapist can improve speaking and help the patient to communicate more effectively. A speech therapist can also help patients with swallowing related disorders. A speech therapist will create a customized treatment plan based on the individual needs of each patient.
Care of the orthopedic pre (&/or) post surgical patient includes all levels of lower and upper extremity injuries and conditions. A specialized program following amputation, hip or knee replacement, is designed by our physician and instituted by our director of rehabilitation. Early ambulation is encouraged as it helps patients gain confidence and competence following injury. With the aid of our MASS system, (see Neuro rehab), patients have the confidence to attempt walking without the fear of falling.
